I confirm that to the best of my knowledge, the information which I have provided to Heritage Health Food, Inc. (hereafter “HHF”) in my employment application is accurate and I understand that HHF will rely on it to evaluate my candidacy and qualifications for current & future employment opportunities.

I agree that HHF may use the data provided on my employment application data such as name, address, work history, etc. and submitted for employment consideration, either online or in person, and acknowledge it is for recruitment purposes only, except to the extent permitted or required by applicable law. The provided data may be processed when creating an employment file by HHF human resource groups as well as by third party providers (e.g. IT system suppliers, compensation and benefit program providers, recruitment process outsourcing companies, etc.) and I hereby consent to the processing of my personal data as described.

I agree that my personal data may be kept on file for up to three years and will be used to evaluate my candidacy for periodic job openings, in accordance with applicable laws. I further agree that my application data may be transferred or shared with third-party providers, regulatory authorities or as otherwise required by law, if necessary for the above specified recruitment purposes, or other purposes permitted or required by applicable law, so long as HHF will take all necessary precautions  to ensure that my personal data is protected through adequate safeguards. HHF will not otherwise share my personal data unless required to do so by law and will not be held liable in the unlikely  event there is a data breach caused by external sources.

Finally, I am aware that I can at any time elect to edit my personal information or totally revoke my consent and purge all of my personal information by emailing contact.compliance@heritagehealthfood.com. I am also aware that if I formally withdraw my application via phone or via email to the above address, I will no longer be considered for employment by HHF.
